The Care Manager Case Management System is a case-oriented patient management system harnessing dynamic form capability created by Community Care Systems, Inc., a US-based company developing software solutions for healthcare providers. 

Promote is a case-oriented school-care management system created by Community Care Systems, Inc., a US-based company developing software solutions for healthcare providers. 

The application enables organizations administering health care programs to design and collect various forms and survey data from health clients in conformity with the ever-changing collection requirements of each care program (>200 different collection forms at any time, usually funded by government organizations).  

The form/survey designer provides an instant preview to view and test the form/survey as the designer makes changes. Designers can create complex forms and sub-forms that tie into patient and carergiver records to validate, autofill, and calculate fields as required. The form designer auto-designs the form layout and provides a print preview for each form design; however, these can also be tailored where needed. 

The application includes pervasive generic reporting capabilities on patient and program demographics and highly customized reports for cost-recovery invoicing, longitudinal analysis, letters, supporting documentation required for patient legal services, etc. All dynamic form data can also be exported in various formats. 

Existing customer data was migrated into the Care Manager care program to avoid interruptions to ongoing business functions throughout the migration process, allowing the end-user organization to transition with minimal retraining and risk.  

The project supports 30 care programs, with more than 200 dynamic patient data collection forms, over 350 carergivers and staff users, and over 75,000 patient/case records.
